Key Safe Release

Key Safe Release: I authorise Hazelcare Limited to hold the Key safe number for my property. I understand that they will only use it if an emergency arises or I have given permission for the care assistant(s) to enter my property via the key safe. The key safe number will be held at the registered office of Hazelcare Limited.

Data Protection Authorisation

Data Protection Authorisation: Hazelcare Limited will be inspected by the Local Authority, Primary Care Trust and the Care Quality Commission who may need access to my file. I authorise Hazelcare Limited to permit my file to be accessed, in accordance with the Data Protection Act 2018 (GDPR). (Please inform Hazelcare Limited of any individuals you do not wish to share your information with).

Medication Administration

Medication Administration: I authorise Hazelcare Limited to assist me with the administration of my medication. I understand that they will only assist me with medication that is prescribed by my GP or my pharmacist.

Urgent Medical Care

Urgent Medical Care: I authorise Hazelcare Limited to administer First Aid, call my GP to visit, call for an ambulance/paramedics to visit when required and arrange for me to be taken to hospital/ A&E unit when necessary.

Prescriptions

Prescriptions: I authorise Hazel care to obtain prescriptions for my use where necessary.

Finance Assistance

Finance Assistance: I authorise Hazelcare Limited to assist me with the management of my finances. I understand that they will only assist me with my finances at the request of either myself, my advocate or other health and social services professional.

Consent to care

Consent to care: I authorise Hazelcare Limited to assist me with the care as agreed in my care plan. I understand they can only follow the agreed care plan.

Risk assessment

Risk assessment: Hazelcare Limited have completed a full risk assessment to support my changing care needs. I have participated in each risk assessment. I fully understand and consent to the need of changing these as and when required.

Pictures: I authorise Hazelcare Limited to take pictures for the Care files
