Hazelcare Supported living Model
This model is designed to guide individuals through a structured pathway towards greater independence while ensuring they receive the necessary support at each stage. Hazelcare offers a personalized approach to supported living, empowering each individual to develop the skills, confidence, and connections they need to thrive.
Description:
Clients begin their journey in shared housing, a supportive and community-based environment that fosters social connections and daily routines. Here, individuals can adjust to living with others, receive basic support, and build confidence in a shared setting.
Key Features:
Community-Based Environment: Shared accommodation offers a sense of belonging and community by connecting individuals with housemates and staff.
Routine Building: Staff help individuals establish daily routines, promoting comfort and stability.
Basic Skill Development: Staff provide assistance in daily tasks like cooking, cleaning, and managing personal care to encourage independence over time.
Objective:
This stage focuses on creating a comfortable, community-centered environment where individuals can feel supported as they adjust to shared living.
Description:
After clients become comfortable with shared living, they transition to a smaller shared property. This step offers increased independence, as individuals learn to manage more aspects of their daily lives. Staff continue to offer support but with a stronger focus on goal-oriented development.
Key Features:
Enhanced Independence: Individuals take on more responsibility in day-to-day tasks, such as managing their schedules, self-care, and meal preparation.
Goal-Oriented Support: Staff work with each client to set and achieve personal goals, such as building social skills, organizing their space, or budgeting.
Skill Development Focus: Emphasis on skills like time management, organizational habits, and self-care practices.
Objective:
This phase aims to prepare clients for individual living arrangements by helping them achieve a higher level of autonomy and reinforcing their daily skills.
Description:
Clients ready for a higher degree of independence move into single accommodations with Hazel care’s flexible, lighter support. This stage focuses on guiding clients through the Home Choice process, which includes assisting with applications, exploring housing options, and preparing for independent living.
Key Features:
Independent Living Preparation: Clients manage their daily activities with minimal support, building confidence in their ability to live alone.
Home Choice Support: Hazelcare staff guide clients through the Home Choice process, helping them understand housing options, complete necessary applications, and prepare for the responsibilities of maintaining their own space.
Resource Access: Staff offer resources on budgeting, organizing, and managing household responsibilities to ease the transition.
Objective:
This critical step helps clients feel confident and well-prepared for living independently by ensuring they have the skills and resources needed for self-management.
Description:
The final stage of the Hazelcare Supported Living Model is Independent Living. In this phase, clients transition to their own housing with minimal or optional support from Hazelcare, as needed. The focus is on empowering individuals to live confidently and independently, knowing they have access to resources or assistance if necessary.
Key Features:
Full Autonomy: Clients manage their lives independently, including daily routines, budgeting, and household management.
Minimal Support: Hazelcare provides optional support if requested, ensuring that individuals can maintain their independence with a safety net if needed.
Access to Ongoing Resources: Clients have continued access to Hazelcare resources and support networks to reinforce their independence.
Objective:
This stage represents the successful completion of the supported living pathway. Clients are equipped to live confidently, with the skills and resilience to maintain their independence.
