The transition process from children’s to adult support services should start around the age of 14, meaning you can take your time in researching the different support options available. We’ve seen countless times how valuable that extra planning time can be when making such a big decision - along with starting your search early to look at the different types of homes and locations available, and to understand what might suit your loved one best.

Timely communication
Good communication is equally important, and is something we pride ourselves on at Hazelcare. There are lots of different people involved in the transition process, and so by ensuring everyone is kept up-to-date with timely information, we’ll keep disruption to any existing routines to an absolute minimum. We know flexibility is key, and we’re happy to adapt our approach with your changing needs or preferences in mind.
